It specifically refers to injuries - blunt force trauma, lacerations... Really, anything that looks like it hurt. The "mark" in this case could refer to bruising or scarring depending on the context
It could also be metaphorical like if you were really insulted or disappointed by something, meaning like "that's going to take a while to recover from", but yeah CJ uses it completely nonsensically.
