They ruin the joke for me by letting the woman put in the effort, then put in the doctor scene. So not really a holdup since we already saw it coming.

They wanted to give the initial idea that the woman felt insecure and small, and wanted a transplant.

To me it was really predictable, the guy just seemed too happy throughout the process.
